A number of partners* are working together to apply for funding to get people more active and more active through football. In this survey we would like to know how you feel about physical activity and football, and what would encourage you to get more involved.


This will help us to understand more about the kinds of activities we could offer.


It doesn't matter if you have no interest in football, we'd still like to hear from you.


We are particularly keen to hear from 16-19 year olds and the Black, Asian and Minority Ethnic (BAME) community. You can also complete this survey on behalf of someone else who falls into one of these categories (if you are a parent, carer or support worker for example).


The survey takes less than 10 minutes to complete and, as a thank you, you'll be entered into a prize draw to win a £50 Love2shop voucher.


The survey is being managed by an independent researcher who complies with all GDPR and UK legislation on data protection and follows the Market Research Society's Code of Conduct. That means that all responses will remain anonymous and will be treated with the strictest confidence. Your personal details will only be used to contact you if you want to be involved in the project, or if you win a prize.
